Lighting Mods has one of the most thorough guides to cleaning DSLRs I've seen. I've yet to clean my sensor despite having my camera almost 2 years now. Maybe if I shot more landscapes I'd notice the spots more but they rarely are ever noticeable.
If you do plan on trying be sure to have a very good guide such as this on hand as you can really trash your camera if you do it wrong.
